Emails show tracked in Outlook but aren't in CRM

All,

Had a seemingly random issue show up in our Dynamics CRM (On-premise). We're using server side sync to Office 365 for email.

In CRM for Outlook (not the app), an email will show tracked to an entity but nothing shows up in "Closed Activities" on the entity. Clicking on the link in Outlook will open the entity in CRM (ie from Outlook everything looks like its successfully tracked).

Doing an advanced find, I can find the email but "regarding" is null.

Any ideas on troubleshooting?

    I'd say it could be a security thing why the activity does not appear on the activities. When you locate the activity in advanced find is it Closed?  Also, if you add the "regarding" does not contain data, does it still appear in results?
