Upgrade from AX 2009 & 2012 to Dynamics 365 Finance & Operations

Dear AX Experts,

Prospects are inquiring if it is possible to successfully upgrade all their data residing in AX2009 & 2012  to the latest Dynamics 365 Finance & Operations?

Is upgrade the best practice or is it better to do a fresh installation of Dynamics 365 Finance & Operations.

If via fresh installation of DFO - how to migrate over data from AX instance to DFO instance?

Please advise.  Please provide some documents about this requirement.
