AX 2012 Creating duplicate production work order (inventory)

Suddenly the system is duplicating WO's which is creating inventory 2x.  Any suggestions on what is happening or a fix?

    Did you have customization on your system? if yes , it may caused by the x++ query, if not, you can try to reset your personalized form.

    Can you explain from what form you took the screenshot? Without the form name and/or column names it is hard to guess what you shared.
