2. I created an email with a button that links to a Customer Voice survey that has personalised questions (i.e. includes the first name in some questions).
3. I have NOT ticked /Make survey anonymous/.
4. I added the email to a journey.
Result: When the email is sent via the journey it is not personalised and the response does not link back to the contact record.
Question: what do I need to do to make the survey personalised when it is sent via the journey?

Check Email Configuration: Make sure that in the email body, you have used personalization fields properly to insert personal information, such as customer name. Using symbols or special characters can affect personalization. Check again to see if these characters are present.
Check your Journey Settings: In your journey flow, make sure emails are added in the right places and don't have any rules that reduce personalization performance.
