I have a Customer Voice Survey being sent via an email template and triggered by Power Apps Button Send Survey. The button works and it is sending an email but it is not the personalized email template I created and saved in Customer Voice. The email does have the correct survey link. Here is a screen shot of the Power Apps Button flow showing the Email Template choices and the template I created in Customer Voice for this flow "Completed Project Followup" is not listed as a choice.
The email template is listed with the other templates in Power Apps
How do I get my flow to use the correct email template?

I opened a support ticket on this and the support staff worked with me on several troubleshooting steps without resolution. They then did some work offline and resolved the issue.
You experienced some issues while using the send an email Template in Customer Voice.
"On investigating from the backend, we saw that there were some incorrect settings for the affected Survey which was fixed from the back end."
