From what I have seen, it appears that the "Bottleneck" field is not being populated/marked when I look at the availability by BOM on a sales Order line. It appears that this should be showing which item is the first limitation, but in this example, there is no bottleneck marked. Is there something I am missing in the setup?

The Bottleneck field specifies which item in the BOM structure restricts you from making a larger quantity than the quantity that is shown in the Able to Make Top Item field. For example, the bottleneck item can be a purchased component with an expected receipt date that is too late to make additional units of the top item by the date in the Needed by Date field.
In your case looks like all materials are available to make 55 of 11801-20 by 1/31/2022. It is looks strange to me but looks like you have all materials.
