CRM 2016/365 on-premise - missing home tab on form design and security menu from Settings

I only see the File and Insert tabs when in From design - there is no Home tab. The security Menu is also missing. I have added the recommended entry to the sitemap for the security tab and it still does not show. I have also viewed the application ribbon in the workbench and the FormEditor.Home.Tab is there but still does not show in design mode. We upgraded from 2013 to 2015 and then to 2016. Everything else works.

Alex, Definitely a good thought. I tried with both IE and Chrome, I cleared cache and I tried on several machines with 2 different system admin logins. It just does not appear.

some kind of cache issue maybe? did you try another browser/private session?
