Connections not showing up on Account Page

Hey there,

I have created a form where I would like to be able to see who my customer sells to and on what credit limits they have issued to their respective customers.  I am looking to do this via the Connection functionality and to have it shown in the Account tab of my client.  I created two custom fields and have them inputted correctly as far as I can see, but I can't seem to get the data to show up in the report - it only seems to capture the Name / Relationship / Details and does not display the captured data from my two custom fields.  I have some screen shots included here.

Step 1:  Create a Connection

So here is what I have done for creating my connection

Step 2:  Click Save and Close and then go back to the Account screen and this is what I see:

You will notice the two custom fields are empty, even though I inputted 500 and a date and clicked save.

Some assistance would be great to get these fields running; I would prefer not to have to use the Description field to blend values and dates via user input into an open text block.
