Can you please specify the version of Dynamics CRM when posting a question?
That will help provide you a proper solution based on your environment such as Dynamics CRM Online or CRM 2016 (if On-Premise).
If you are on CRM Online, the easiest way as mentioned is using VOTC (Voice of the Customer), which case be added from Dynamics 365 Administration Center.

Which version of CRM are you using?
Option 1:
If you are on the Online version, you should be using the Voice of the Customer for Dynamics 365. You would need to install this application into your tenant from the CRM Admin Portal.
This allows you to create and send out surveys, as well as analyze the survey data easily.
Option 2:
If you do not have the Voice of the Customer application, and you are using O365, then an easier workaround is to use Microsoft Forms.
And then, use MS Flow to push the response data back into CRM.
I would prefer using Option 1 over Option 2.
Option 3:
Use a 3rd party solution such as ClickDimensions Web Forms.
The Web Forms help you to create surveys, have a separate URL for survey site, capture and analyse the responses.
This works for Online and OnPrem as well.
