It is quite strange that the issue is gone when you try to confirm multiple times. Or it should confirm directly or always fail. Are you aware of any differences between the test and staging environment? E.g. security roles?
If the settings are the same, then you can create a ticket for Microsoft Support like Ludwig mentioned.

yes we have enabled. anything will be related to this ?
After completing the workflow, Form never been updated/Refreshed.
We have tested with different users, we are able to reproduce the same issue for every user.
In test environment we don't have any issue
In staging we have issue.

3. Clicked on the confirm - I should click 2, 3 times to get PO confirm.
