The issue usually occurs after an upgrade, but not always.
I would still run the scripts mentioned in the link and see if that resolves the issue.

I was in the process of running SmartList reports. It was not associated with an upgrade.

When did this issue start? Usually we see it right after an upgrade, with some records missing due to the old clearcompanies script.
