can't add Child organization to parent

I am trying to add a Child organization to a Parent Organization. However, I am not able to add for some Child organizations (image 1). I found that

when I create a child organization with the Relationship type field value = "Client", then I was able to add (image 2 - refer to the test 2 record ). However,

If i choose any other values from the Relationship type dropdown list (image 4), i am unable to add (image 2 - missing test 3 record).

Could you please advise me on where I can see the error log? is any suggestion for the reason behind this issue? Thank you, Siva

I fixed the bug.

the reason for the bug was that the view used for adding "Add Existing Organization" had a condition to display records where RelationshipType = "Client". Once I removed the condition from the view, I was able to add any child records to the parent.
