WF does not trigger upon record field changes but able upon record created

Hi Guys, i really need some help here as i am stuck for about 1 week. I am trying to trigger workflows whenever the record field changes at the follow-up date. whenever the follow-up date is changed, an timeout will start counting then email will be send.

Can anyone give me helping on this?

I have attached the WF too!

Hi,

Have you selected the 'Follow-up Date' field in 'Record field change' list? If yes, select other fields and check whether workflow triggers or not.

Also unselect 'Automatically delete completed workflow jobs' option, if it is selected, during troubleshooting this problem.
