Error when creating a new Item

I am not able to create new items from scratch, however, when I copy from an an existing item to create a new one , I have no problems.

error message:

The Item does not exist. Identification fields and values: No.='1234'

gdrenteria is right, from the error message this is from Premium Soccer Customizations by ArcherPoint, Inc, please contact this partner or uninstall and try again.
