How to add multiple forms to the same entity

Hi everyone,

Every time I try to create an additional Main Form for the same entity, I am navigated back to the existing Main Form. I have tried to do this on different environments/accounts, but the same behaviour occurs.

I am not sure if I missed something obvious?
