With fields that the program offer, the case isn't create.
I found out that if I insert the name of an ACCOUNT, the case is automatically created correctly..... but not with any account and this is very hard to understand, because in my test sandbox all accounts are created with only the column "account name" compiled.
Can somebody give me some idea or suggestion where I'm mistaken?

Can you attach one screenshot of one dummy case created?
Full page screenshot? Want to see ACCOUNT which you referring to, will it be a part of the case/case form?
