Hi,
We created an entity form for the contact entity, we have also created an entity permissions record and assigned the required web roles. We then created a web page and linked the entity form to it. For some reason, the form is not showing up on the portal page.
We tried the exact same process on the Sandbox environment and it worked.
Are we missing something?

You can check following points:
1.Remove the Entity Form Metadata to test.
2.Check your Entity Form mode
‘Insert’ mode expects no input parameter and ‘Edit/Readonly’ mode expects the record GUID
3.check Entity Permissions?
And compare the differences with those in a sandbox environment, also you can try to clear browser cache or change another browser or privacy mode to test again.
