    You can pass the id of the record through the URL and get the information using WebApi.

    Hi Sunil,
    First you create a JavaScript webresource with a function (say, onButtonClick()) which will be called on button click action.

    Then download and import ribbon workbench solution to your development environment. This solution helps you to add ribbon button to entity ribbon, set related action and rules.

    Go through below links to understand how to add a button. Add action to this button to make a call to a function created above: onButtonClick().

    Create an HTML webresource which contains HTML content as per your need. Also add JavaScript reference to link to websource created in first step.

    Inside onButtonClick() JavaScript function, call built-in function to open HTML webresource. Refer below link.

    Read these articles to prepare your HTML webresource which fetches data from the form.
