Is there a reason embedded forms expires on external webpages? see below.

How long has this form been created? Would it be possible that the form has been hosting on webpage for a long time?
The error message can be found in source code of form-loader.js, and its error type is "CorrelationExpired".(It seems that it is relevant to cookie expiration.)
Try to create a new embedded form to check whether it would work.

Hi, There has been an update to the form scripts in May 2020, and the path of the form script should look like this: mktdplp102cdn.azureedge.net/.../form-loader.jsTherefore, make sure to update that in the HTML and the form should work properly.Here is more information: https://docs.microsoft.com/en-us/dynamics365/marketing/developer/marketing-form-client-side-extensibility#javascript-api
