We have a PO which we completed the approval process but showing the status as "In external review".
Our workflow is fine and nothing such we can see in other orders. Only for one order we are getting this.
We do not have any "external" review system.

Have you checked in Purchase inquiry in the PO if this is generated or not?
If you have generated this, then you will get the journal for this in 'Purchase' section in the top bar. And at the same time, the approval status will be updated as 'In external review' as you highlighted.

Thanks for super quick response!!
I can see a purchase inquiry journal is available. When I open it; it looks like the purchase order confirmation which I can take a print copy.
But we do not use it. I guess by mistake the client has generated this.
Can you tell me what is the use of it??

This is you can consider an additional review process where you are expecting a vendor's review and acceptance of the PO before you confirm the same PO in ERP. This is not mandatory to use this feature but if your company is following any policy where supplier's review and acceptance is involved; then you may have to follow this.
Once you got the confirmation from the vendor, then simply Confirm this PO and you will find the approval status as 'Confirmed'.
