Hello
Please I would like confirm if its possible to create a custom form in Dynamics 365 Customer Service Module, without a Status Code, Service Stage and Status Reason.
Specifically, as per screen shot below, is it possible to create a custom form without the highlighted sections and how?

Hello,
Please follow below process which will help you to implement your requirement.
A . if you want to remove BPF for a specific model driven app during record creation then follow below step.
1. Go to a solution and open model driven app where you have case entity. Normally it is in customer service hub app.
2. Then go to the BPF and uncheck the given BPF.
Then Publish the application and once it is publish you will able to see that BPF is removed from the case form for that specific model driven app.
B. If you want to deactivate the BPF then just got the solution and open BPF.
Then Deactivate it. Once you will do it then it will be not visible for new records.
But for old records it will be visible. If you want to remove for the old process then follow below article steps.
