I have installed Management Reporter on the server and the application pool nor the website have been created. I have the proper permissions. Has anyone seen this before? Thanks

Are you using Management Reporter 2012? As far as I know MR 2012 no longer uses IIS. It now run services on the server. These are the application service and process service. If you launch the Configuration Console you should see these running. Have you tried launching Report Designer to see if it will load?

Management Reporter 2012 no longer uses an IIS website so you will npot see an entry there.
Open the Management Reporter Configuration Manager and it will show you the two processes and if they are running.

I am getting the error message 'Couldn't retrieve the list of databases: The connection attempt failed. Any ideas?
Sounds like the user you're using does not have access to SQL Server.
It does, I see the user it is the owner of the Management Reporter database. The user has adm rights over all dynamics databases
What version are you using?
SQL 2008 and Management Reporter 2012
What rollup?
