Voice of the Customer required permissions

I'm having an issue getting a user the correct permissions to do all of the Voice of the Customer functions (survey creation, preview, etc). He keeps getting a business process error. It's permissions related because the errors disappear when I temporarily grant him the "system administrator" role. 

I've given him all of the "survey" related roles: Survey Administrator; Survey Designer; Survey Feedback Publisher; Survey Service; and Survey User. But those don't appear to be enough. Is there another role that I'm missing?

We're using Dynamics CRM 2016 Update 1 Online at this time.

    From the Error trace provided by you, we can see that this error is due to Missing read privileges of Image Token cache entity.

    This issue should resolve after giving the read privileges of Image Token cache entity to the Survey user role and assigning that role to the user.

    Thanks for your reply but adding that permission to the role didn't change our situation. 

    I also am seeing a Business Process Error dialog (e.g. when trying to preview our first survey), but it has no downloadable log file. It only shows this info. I have the CRM "system administrator" permission which yesterday allowed me to preview a survey.

    Below is an error that the user sees when trying to get to a VoC stored image. We're very new to the VoC features and are testing it in our sandbox environment.

    Not sure what the exact error you are getting.

    We also faced some issues related to the permissions of Survey.

    We gave the Azure deployment read privilege to the Survey user role and gave that role to the user.
