I have the following scenario

i want the user to fill a questionnaire that includes taking images for every customer he visits.

the task must be performed with mobile offline capability

can this be done using field service?

    We have done something similar.

    We created a custom entity and made our own fields. We then made a security role so the correct people can use it. All you have to do then is go into Woodford and enable the entity and fields so that they show on the Mobile Device.

    As for images. They can just add them into the notes or activities section.

    the mobile device is it Resco mobile or field service mobile appication ?

    One more question.

    If this was built using custom entities. then how are you going to support multiple surveys with multiple questions (The admin user should be able to add, remove, and modify these questions and surveys)

    You could probably create a new custom entity called 'Questions' where you could have a list of Questions to choose from. You could apply security roles on the entity so only certain users can access them. 

    You could add fields to the entity and then use Form rules in Woodford to display and hide certain fields depending on the question displayed.

    eBECS actually built a tool specifically for this called the Site Survey.  This video is of an older release of it, but it does what you're asking, and supports offline: ebecs.wistia.com/.../p1i6a6l5t4
