Filter the multi select lookup field based on lookup field selection

Hi All,
 
I have created a form , in that form one field (PO-Lookup field) and other field(item number-multi select lookup field) .
Here,if i select a PO from lookup then it will filter the item number field related to the Purchase Order.
 
Please help me ..how we will do this ...

    Can anyone suggest me the exact solution...i was trying to complete it but it has not working as expected.i am still trying to complete, but not getting the exact way for the solution.
