Business Portal (5.1 Sp3) Executive Center site has 404 page not found error after upgrade

Navigating within the Business Portal site and when attempting to access the Executive Center site page, I get a 404 page not found error. The relative URL is BP/Executive and the page is attempting to load BP/Executive/Executive%20Center%20Common%20Pages/Executive%20Center%20Home.aspx.

Business Portal was upgraded from 4.0.2 to 5.1.3 on a new SharePoint Foundation server. The wss_content database from v4 installation was upgraded into this new environment. No errors related to features not upgrading or failing were reported during the upgrade.

From a troubleshooting perspective, I have been able to determine that the site does exist and I can navigate to BP/Executive/_layouts/settings.aspx to view the Site Settings. If I view All Site Content, I have a document library named Executive Center Common Pages. Within this library, the Executive Center Home.aspx document does exist. I noted that the date of this document was old and did not appear to have changed with the upgrade. In comparison, some of the other Center pages have newer aspx files so I suspect that something occured to prevent this from upgrading. Another test that I did was to try and reinstall the BPExecutiveCenterCommon feature from a SharePoint perspective to see if this would correct the 404 error. When I tried to deactivate the feature, I got the following:

Feature 'bd5bd245-bd0f-4a10-8b66-776465b4b218' is not activated at this scope.

I then tried activating the feature and received:

Failed to instantiate file "Executive Center Home.aspx" from module "Executive Center Home": The specified list does not exist..

For the next steps, I performed a feature uninstall and install operation, both which completed successfully. Another attempt to activate the feature returned the same error regarding specified list does not exist.

Any thoughts on getting this Executive Center page upgraded and working?
