We sometimes change the description of an item at the time of the sale. I'd like to be able to run a report showing those changed descriptions on my detailed sales reports. Any ideas on how to add that to my custom report?

The only place the changed name is store is in the electronic journal table as part of the store copy of the receipt. You would need to decode this receipt and then parse the receipt prior to being able to retrieve the information. This process is not disclosed to partners or customers.
So basically you can retrieve this information to run a report against.
