Unexpected Behavior in Dynamics 365 Sales Workflow

Hello everyone,
I recently created a custom workflow in Dynamics 365 Sales, but it seems to behave inconsistently. Sometimes it triggers correctly, other times it doesn’t, even with the same conditions. I’ve double-checked the steps and dependencies, but I can’t figure out why.
Has anyone experienced this kind of issue before? Any tips on debugging workflows or ensuring consistent execution would be greatly appreciated.

    With this little information, i would suggest it debug the custom workflow activities.  below link will help to debug
     
    Also, check below before start debugging if something unusual there: 
     
    Trigger Conditions
    If the workflow is set to run on change of specific fields, ensure those fields are actually changing.
    For on-demand workflows, check if users have the correct permissions.
    Asynchronous vs. Real-Time
    Asynchronous workflows run in the background and can be delayed or skipped if the system is under heavy load. Real-time workflows execute immediately but can fail silently if conditions aren’t met.
    Scope Settings
    If the workflow scope is User, Business Unit, or Organization, ensure the triggering record falls within that scope.
    Concurrency & System Jobs
    If multiple workflows or plugins are acting on the same record, conflicts can occur. Check System Jobs for failures or cancellations. Go to Settings → System Jobs. 
    Filter by your workflow name. Look for Failed or Canceled jobs and review the error details.
    Security Roles
    Missing privileges for the user who triggers the workflow can cause intermittent failures.
