How to know who has done customizations to the screen in sl7.0 and how to revert back the customization for the screen to the original.

You can see what customizations have been applied to a screen and the level of customization by looking at the customvba and custom2 tables in the system DB.

Please refer to chapter 3 - page 3-8 of Introduction to Customization Manager in Microsoft Dynamics(R) SL 7.0 (Training Materials for course #8965A). You can web search "Course 8965A" to get the link.
I recommend going through the whole training if you are new to customizations, but this one page should answer your immediate questions.
