Adxstudio Portal bug? - Note Upload works on initial load of Entity Form but control is not there on submit when "On Success" is set to "Display Success Message"

I have an Entity Form that is surfaced to a Portal web page.  On it is the option to upload a file using CRM Notes (annotation).

This worked great until the client didn't like the fact that when they submitted the form they were taken to another page.

I changed the relevant Entity Form record's "On Success" field from "Redirect" to "Display Success Message".

Now when the web page loads the control to upload documentation is there and works.  A new issue though, when you submit the form, you get the success message but the control that allows you to upload documentation disappears.

Is this correct behaviour or known issue?

    This wasn't checked.  Even with this unchecked, the control should still appear if a document was uploaded showing the name of the file.  Even so, I checked this and retested and the issue still persists.

    Attach File is unchecked.  This is because I don't want the control at the bottom of the form.  This should be okay.  A Note control is on the surfaced Entity Form on one of the Tabs.

    "CRM Note Attachment" is selected for the "Attach File Storage Location" option.

    I'm wondering can anyone recreate or is it just me?   On Success on the Entity Form must be set to Display Success Message.

    There's a "Allow Multiple Files" option.  Is this enabled?

    "Allow Multiple Files - Boolean value indicating whether or not the user can upload more than one file."

    Hi again,

    I've edited my post above to include the images.  You can see in the 2nd image the control is gone.  Only occurs after submitting the form.

    Thanks Nadeeja, but the client wants the form to remain visible, which it is.

    It is just the upload documents control that disappears.  The other fields on the form are there and editable, which is correct.

    The control to upload documents is gone.  Refreshing the page shows the upload documents control.

    Image 1: Form on initial load.  You can clearly see the control that allows the user to upload supporting documentation.

    Image 2: When the form is submitted, we want the user to remain on the page with the form visible.  They may want to make further submissions.  For some reason, the control that allows users to upload supporting documentation is gone.  This behaviour only occurs when "On Success" is set to "Display Success Message" and the form is submitted.

    Hi CVO,

    There's a "Hide Form On Success" field you can untick.  Once unticked, you should see the form after form submit.
