Populating states related to a country on a custom Entity form, States should be multiselectable

On a custom Entity form user selects Country (probably a picklist or a lookup) related states should be multiselectable for the user. Depending upon Country selection user should have the provision of selecting more than one state. what are the possible design options?

 

As OOB multiselect feature is not present, though of having states displayed in a subgrid on the custom Entity form, but not sure how do I filter subgrid records depending upon Country selection
