Hi JPM00,

Can you provide more background information? It will be helpful to know if there was a recent change to this environment or not. As mentioned by Ramit, possibly the action center has the details of why the form couldn't be opened.

Did you have any new customization?

1-Also try to do full build and Sync the Database.

2- restart services.

3- Try to reset usage data

Is it a tier 1 environment or tier 2?

FOr Tier 1: ry to rebuild whole packages and restart the VM

For Tier 2: Try to restart the services from LCS
