Hi,

I added some attributes in contact enitity, for example -

Referral Date (Hidden field) [include current date]
Your Affiliation: [Select One: Insurance Carrier/TPA, Employer, Other[RD1] ]
Your Company: [text entry]

Also added one more custom entities [Referral Contact] and want to show only some of [refferal contact] entities form fields based on selection of [Your Affiliation] contact entity attribute in first step of web form.

Please guide me how to achieve this and what would be best way to do this entity forms or web form. I need to insert data in 3-4 custom entities using customer portal form.

Best to start by watching this video, it refers to v7 but the basic concepts will be the same in v8. From there you should be able to ask more directed questions on the implementation challenges you might be encountering.

If the end goal is to edit multiple entities in a single flow web forms are definitely the best option.
