I am currently in requirement gathering phase in for Finance and operations IOM implementation and would like ask if anyone out there have questionnaires for requirement gathering.

    You can use the attached implementation guide I have put together, the first section contains Key Decisions and a Checklist of the Implementation you can use to build out requirement gathering.
