Intermittent Quick Create Save button

A quick create form for incidents works fine when accessing it via the account form, but when accessed from a contact form, the save button is no longer firing any events. I created a script to handle OnSave for the form and just put a 'debugger;' statement in there to get a break point. I hit the breakpoint from account, but not from contact. I'm fairly new to CRM so I'm not sure where to even begin with troubleshooting this. I've asked another dev about custom scripts that might be interfering, either from the page or the ribbon, but they cannot find any issues.

Any ideas? TIA. This is an on-premise install, if that matters.

    Ones regarding unsafe "Content-Length" header from the global.ashx, but nothing that is related as far as I can tell.

    version of CRM? also make sure you have only one Quick Create form

    Dynamics CRM 365 v1612. I think we may have found the issue. Inconsistent entity relation naming. :\ 

    Edit: Or not... The search continues.

    Have you tried the same on all the browsers? Could you try on different browsers and see if you still have the issue. And could you please share the version number as well.

    I have tried on the following onprem version and it seems to work fine from Account and Contact.

    I have tried on Edge, Firefox Developer Edition (which really breaks when trying to debug), and Chrome. All exhibit the behavior. Dynamics CRM 365 v1612. There is nothing in the console log that shows anything helpful. I've tried tracing into the scripts, but I see nothing obviously wrong.

    The work-around is that we disabled the ability to create an incident from a contact page. We will eventually need to solve this, but the project is in UAT, so my ability to debug is thwarted for the moment.

    The same form works from the Account page. My thought is, and I've shared this with the developer that has been working on this project, is that some customization in the Ribbon has broken the Contact view somehow. I'm only assisting with debugging this. This had previously worked back when I was more active on the project. The only thing that changed between then and now has been Ribbon customization. I'll keep everyone informed of the status as I go. Thank you all for your input. I appreciate it greatly.

    Thanks for the update

    Its likely its a bug in V1612 but its worth checking the custom script to make sure it does not touch or block any OOB js functions or styles

    Let us know once you found the solution as it would definitely helps other users as well.
