Field not updating as part of Workflow

Good morning everyone!

We have a workflow that updates a lookup field on the Work order form, once a case is converted into a Work Order.  This field was being updated without any issues until last week.  No changes were made to the system, nor the workflow.

The issue is; the field no longer automatically updates the lookup field until after the form is changed.  When I convert a case into a Work Order, the lookup field is blank.  But when I change the form, the field updates itself.  It's almost like the form needs to refresh in order for the lookup field to be updated.

Any ideas?  I've pasted a screenshot of the Workflow below.  As I have said previously, the issue doesn't seem to be with the workflow itself, as it has been functioning properly until recently.

    Can you take a Screenshot of the entire definition of the Workflow? I mean, the entire window

    The workflow is initiated upon record creation. Below is screenshot of the complete Process properties and the definition of the workflow as well:

    You have to make it a real-time workflow. Otherwise, it may take some time(up to a few minutes) before the workflow actually starts.

    That was it.  

    We've never used Real Time workflows before.  Great learning experience here.
