Business Central does not have a single standard “Item Revision/Version” field, but before customizing I would determine what “version” means operationally. If multiple versions of the same item can physically coexist in inventory, the standard Item Variant functionality is usually the best fit: Variant Code is carried through sales, purchasing, production, inventory transactions and Item Ledger Entries, and you can even use SKUs to assign different Production BOMs for different variants. If instead this is an engineering revision that becomes effective from a certain date, standard Manufacturing already supports Production BOM Versions and Routing Versions, each with Version Code, Starting Date and certification, so BC automatically uses the applicable version without changing the Item master. Therefore, I would not simply add a Version field to the Item Card, because—as you correctly suspected—you would then have to propagate and validate it throughout documents, production, planning, reservations and ledger entries. Only build a custom item-revision framework if the business requires revision-level traceability beyond Variants/BOM/Routing versions; otherwise use Variants for simultaneously stocked versions and BOM/Routing Versions for engineering changes over time.
