Design Function Not Available to User with D365 Extension Mgt Permission

Users named Fats and Vivian have same Permissions (D365 Extension Mgt) but only Fats can see the Design function in the App bar (see image for reference).

Is the Design function limited to a number of users only?

Are the two users on the same environment (sandbox)? Or maybe one of them is connected to a production environment?

Based on your screenshot. For vivian , please try to set company name to blank for permission D365ExtensionMgt
