No its not happening on create. at create it is fine, but when you go back into the form it is there
This did fix my issue with the appointment, however has created another issue with my visual studio built report that is used to collate all appointments that week - now this is coming up with the following html code - but there are no issues with the actual appointment. I am assuming that this is because my report is plain text and now the appointment is rich text its freaking out. Any help on how i am to change my report in visual studios to rich text would be much appreciated.

When i click on HTML Web Resources I only get a long list of resources to add to the form, nothing shows that it is already in the form

Thanks Abdul, its an odd one as the appointment form was created years ago and i have made no changes to it, but its only in the last month that users have been getting the HTML code appear. Do you know how i can check if their is a HTML page or IFram associated with the form? I have had a look and can't see anything out of place

In other words, these are coming from the HTML page (or it may be the IFram) associated with the appointment form.
that code is HTML (as in a web page), maybe the form has been customized? if this is the case you should ask who customized this appointment form.
