How to request user to enter reason for disqualify the lead

Hi,


I need to customize functionality that when user clicks on disqualify button 

that either pop up windows can be shown where user needs to fill fled to put reason for disqualify

or

Workflow that will check if field on entity form for explaining reason for disqualify the lead 
is not filled.

    I dont think you can open custom popup on disqualify button,but you can try below link,
