I have looked through the entities many time and cant see where I can customize the forms which show as ballons.
e.g. When the address line is clicked and balloon opens next to it so I can enter the address details. Where can I find this ?

It's called flyout menu and its not customizable .You can't customize Composite controls like fullname and address fields.
