We have configured a portal redirect which takes an inbound URL and redirects to another portal web page. This works fine but when we de-activate the redirect and carry out a portal cache refresh the redirect still happens. Is this by design?

Please try to change the Publishing State of web-page to draft. You can also refer to the link docs.microsoft.com/.../web-page.
Accordiung to this link, The current publishing workflow state of the page, which may dictate whether or not the page is visible on the site. The most common use of this feature is to provide published/draft control over content.

You're right - as far as I can tell, the code doesn't care where the redirect is active or inactive. My guess is it wasn't done on purpose, just wasn't something that was thought about when that code was implemented.
