I have a requirement that user should have an option to select the document type ex.educational,employment etc and then add the attachment.so the type should be linked with the notes attached.

Create a new relationship (1:N) between opportunity & custom entity. Or go to custom entity & create a new lookup field of Opportunity entity - this will create the needed relationship.
Then you can insert the subgrid of custom entity in opportunity form.

You can add field in customizable and custom entities.
Note is system entity hence you can't add field.

Notes entity is not available for customization so you can't add any field on thsi entity. I had the similar requirement and I achieved it by creating a custom entity "Documents" with some fields like Document Type, Document Name etc and enabled notes for that entity. And then I used this entity as a child entity to upload documents instead of using teh Notes directly.
