AP Voucher blank task

An AP Voucher was created and posted with a project but blank task.  We've tried to reverse the voucher, do a project charge entry and j/e to no avail (a task is required).  Any recommendations to fixing this? If tables need updating, what tables (aside from aptran/gltran)?
