Hello,
I need to run a workflow to run every year, for all active account records. What is the best method to do this and how?

The easiest way is to use Ms flow.

Would I say repeat every 12 months, or should it be 11 months? I want it to run on the first of every year. Thank you. Just wasn't sure if this counts the starting month or not

Yes it's correct. Every 12 months.
You can do a Test with every 1 Hour and check the result :)

Will this do all active records or 1 record? I saw the Record Number filled in, so was just wondering. What is the Account Number with Test in int? Is that an actual Account record?
In the example I update the Account Number field with "TEST" for all account records. You can use a filter to get Only the active records.
