I have a customized form in EP on AX environment, when the environment is upgraded, when I clieck to open the from , it is blank , how can I resolve this issue ?

Was it a system provided form you modified or your new form from scratch because in the case of earlier, the upgrade could have caused issues to it.
Also which AX version you upgraded from and to. And is your form and web controls and web parts deployed correctly on the upgraded EP ?
it is a new from from scratch , the system was upgraded from AX 2012 R2 CU 6 to CU8 , form and web controls and web parts all are deployed correctly.
Just as a question , may it be a security , permission issue through role center?
not sure about that, can you debug your form to see if its loading adn where is it breaking if thats the case, may be some error is coming which is unhandled
Have you enabled debugging in web.config ?
