Go to settings >>customisation >>customise the system >>expand entity>> scroll down to phone call>>click on forms>>on the right side double click and open information main form
Or
The second screenshot you have shared, go to the same and click on second tab on the top called customise >>click on form
Hi Abdul,
This form can be managed in Settings-->Customizations. It is under the entity "Phone Call" and you can find the form under "Forms" for that entity.

Hi Abdul, you can find the phone call forms in the following;
Settings > Customisations > Customise the System > Entities > Phonecall > Forms
There will be a main form which is the one your looking for, as well as quick create
