The top navigation is called the 'site map'
If you are working on Dynamics 2016 Update 1 or earlier you can use the XrmToolbox site map editor to add items - see shanewcunningham.blogspot.co.uk/.../use-xrmtoolbox-to-modify-crm-2013-site.html
When you are upgrade to 2016 Update 2 (Dynamics 365) then there is a built in sitemap editor. See msdn.microsoft.com/.../gg309424.aspx

The black area above the Site Map, what term describes it? Clicking on the "+" gives access to the quick forms.
I would like to change the Contact Quick Create form to reference Contact HC Quick Create form.
How do I achieve this?

To customize, simply go to Settings > Customizations > Customize the System. You will find multiple sitemaps under Client Extensions, there is typically one which contains all apps, and then others which are each tied to a specific app.
