Uploading 3D Models on Guides Model Driven App?

On the Guides Model-Driven App, once a new 3D Object is created, I should be able to choose a file to associate with the entry:

Nothing happens when the greyed out 'Choose File' button is clicked, with no explanation.

I am able to upload models via the Guides store App, but then have to manually add them to a collection.

I have the exact problem, and I confirmed with my Admin that I have Author and Operator read/write permissions. I use to be able to upload 3D objects from this window before and out of the blue it disappeared.
