Advanced form with an error in the Portal

Hey guys! I'm experiencing issues with the advanced form in the Portal.
I've followed all the tutorials I've found, and I'm facing the same error.
When I run the site, in addition to the form design being bad, the steps to proceed do not appear:

Does anyone know what to do?
